The issue was MrMC was doing a HEAD request for mime-type, which failed, followed immediately by request for the content. The content 502'ed and failed. Strangely it worked fine when stepping through the code.
The fix was to pull in https://github.com/xbmc/xbmc/pull/9504, which has a side effect of moving the mime-type request much earlier.
So something about fast back-to-back requests to the proxy server. Maybe some sort of DDOS protection ?

fix is in 3.8.1, have you tried Emby server or IPTV strm file?

odd, I just took those xxx.strm files you emailed, put them on my NAS and browse and click on one.
