videotoolbox is a rockstar with h264. seems steady at around 80% CPU no matter the bitrate.

Is it limited to only h264? Are there limitations as to the profile of h264? Interlaced material such as 1080i AVCHD from my video cam?

I have a test file that's remuxed from my Avarar blu-ray that for some reason is using software decode. Remux from other blu-rays done in the same manner are using VTB. I suppose you'll want to see a log?

Only h264, it can handle h264 high profile all the way up to 5.2 level and 4k size :) No h264 interlaced :(

There are rumors of h256 support but I've never found any details.

one dollar says your problem video is VC1. log will tell .

nope, Avatar isn't VC1. ff-h264 for some reason instead of VTB. I'll get the log to you when I get a chance. I've just got a new subwoofer so fooling with it. Previous one broke.
